The following forms must be completed and submitted for your pet to be accepted into our daycare and boarding programs: Client Agreement, House Rules, Pet Profile and Check-In Form. The Client Agreement and House Rules form only need to be filled out once. If any of your pet's important information changes, you will need to re-submit the Pet Profile Form to reflect these changes.

Owners boarding their pet overnight will also have to fill out and submit the Check-in Form before each boarding reservation. Daycare guests will only need to fill out the Check-In Form once unless any of the important information regarding your pet has changed. If so, you will have to re-submit this form with the most up-to-date information.

ASSUMPTION OF RISK: Owner acknowledges and is aware that the employees of unFURgettable Pet Resort & Spa are not veterinarians and do not have a background in animal medicine and are not expected to diagnose or detect illnesses in the pets utilizing Services at unFURgettable Pet Resort & Spa. Owner understands that the service’s leash-free environment allows pets the opportunity to play in close physical contact with each other. Owner further understands that pets are unpredictable in nature. As a result, no amount of vaccination requirement, supervision, sanitation or personalized care from unFURgettable Pet Resort & Spa can be 100% certain to prevent pets from being injured or contracting an airborne virus or communicable disease. Despite these precautions, Owner understands these risks are inherent to the services and hereby releases unFURgettable Pet Resort & Spa, its employees or members/other agents, from any and all losses, damages, costs and expenses arising out of or in connection with any injury, communicable disease, airborne virus, or any other medical condition contracted by Owner’s pet at unFURgettable Pet Resort & Spa. This also applies to any claims for injuries or damages related to such medical care or transport. Furthermore, Owner agrees to be held solely responsible for any and all acts and behavior of their pet while in the care of unFURgettable Pet Resort & Spa, including payment of costs for injury to staff or other animals or damage to our facilities caused by Owner’s pet. *

Owner further understands that pets play and lead with their teeth and injuries including cuts, scrapes, and puncture wounds can be a result of play and not necessarily aggression. Owner understands and agrees that unFURgettable Pet Resort & Spa employees may not notice these nicks or scratches before the departure of Owner’s pet(s), and therefore, might not be notified. Owner agrees to be responsible for all damages and medical treatment for all injuries to their pet as a result of play between pets. Owner also understands and acknowledges that if an injury is proven to have occurred due to unprovoked aggression, Owner of the aggressive pet is responsible for the cost of medical treatment of the injured pet. *

Owner understands if their pet is not already on flea medication, he/she will receive a dose, at a cost of $5, at check-in to eliminate possible fleas. *

Owner understands, if boarding their pet, check-out time is by 12 noon. Pets not picked up by 12pm will be charged a discounted daycare fee. *

Owner understands the lobby is closed to walk-ins on New Year’s Day, Memorial Day, Easter Sunday, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day. *

Owner declares their pet is in good health and has not been exposed to any contagious or communicable illnesses within the past 30 days. *

Owner hereby represents that all information provided in this document is accurate, and agrees to pay for all services and fees herein. *

Owner hereby agrees that their pet’s boarding is subject to the terms and conditions set forth in the unFURgettable Client Agreement that owner has previously signed. *

Owner attests that if they are not the sole owner of the pet subject to this application, the information owner provided is sufficient to enter into this application for and on behalf of any other owner or representative. *
